You have two options for the 2010 Premiums.

      • The first one is formatted as a typical PDF version; anyone should be able to print this and fill it in by hand.

      • The second attachment is formatted as a WORD file [.doc]. If you can open it, it should allow you to fill out the trial application form on the last page on your computer and then print it to mail in.

PLEASE remember that you need to include a signed waiver with your application and cheque.
